Full coverage:

• New patient exam
• Bi-annual periodic exam
• Intraoral complete series or Panorex
• Annual bitewings
• Biannual dental cleanings

Partial coverage:

• Bi-annual fluoride treatment
• Dental sealants
• Fillings, crowns, dentures, partials, endodontic treatment, and oral surgery

The total annual costs are:

• Preventive plan $289
• Periodontal plan $389
